• Educate patients about services available.
• Facilitate a smooth transition and coordinated continuity of care from the hospital to home.
• Provide timely follow-up in the home with support and reassurance through professional health care activities.
• Identify areas of need and formulate a plan of care to resolve needs.
• Provide quality, professional, intermittent skilled care to patients meeting admission criteria.
• Implement a plan of care under the supervision of the patient's physician and involve the patient's family in the care.
• Evaluate and revise the plan of care as needed to aviod patient's re-hospitalization.
• Provide signifacant information to patient's personal physician in a timely manner.
